Shaare Tefilla's Yizkor book will include the entire Yizkor service as well as a list of the names whom we commemorate; their memories serve as a blessing and an inspiration to all of us. Please choose from one of these options if you would like to participate in this project and complete the form below no later than Friday, September 15, 2017.

1. A name listing for $18 per name, with you designated as the one honoring that person.
2. A sponsorship of the booklet, for $180, with an allowance for listing as many names as you wish.
